Personal Care Services

Affinity Care of NJ’s Personal Care services provide long-term care assistance and support necessary to enable aging seniors and infirm adults to remain at or return home rather than stay at an inpatient hospital, assisted living residence or a nursing home.

Personal care services are services prescribed by a physician, supervised by a registered nurse, and delivered in accordance with a care plan. Our personal care services are provided to assist individuals with their activities of daily living, supervise self-administered medications and assist with or supervise the client with ambulation.

Personal care services provided by Affinity Care of NJ include assistance with bathing, showering, grooming, toileting, incontinence, dressing, feeding and transferring a client from one point to another, such as from a chair to wheelchair or chair to bed and vice versa.

Affinity Care of NJ’s Certified Home Health Aides (CHHA’s) deliver our personal care services. They can incidentally also provide other types of assistance including doing light housekeeping, washing the client’s personal laundry, changing the bed linens and preparing nutritious meals.

Our personal care services offer the following benefits:

• A customized care plan recommended or approved by a physician and supervised by our Registered Nurse.
• Assistance or supervision with hygiene including bathing in the tub, shower or the bed.
• Assistance or supervision with grooming, including shaving, brushing hair, brushing teeth, cleaning ears, clipping finger and toenails and dressing.
• Assistance with incontinence (incontinence care)
• Preparation of meals and snacks according to client’s dietary needs and assisting clients with feeding.
• Assistance to client with ambulation and transfers from bed to chair or wheelchair and vice versa.
• Provide reminders to the client to take their medications on schedule and in the proper amount.
• Ensuring the overall safety of the client, including environmental risk mitigation recommendations (such as installing handrails in tub/shower).
